It takes just 5 minutes to complete an application and will allow you to break down your payment into monthly installments. You can either apply at our hospital, where we'll call in your application over the phone for immediate approval, or apply yourself online or you can call (800) 677-0718 to apply over the phone.
You will need the following information to complete your application:
- Doctor's name, or how you plan to use your CareCredit card
- Name
- Address
- Date of birth
- Social security number or ITIN
- Net income
- Housing Information
